AGREEMENT ON ACCEPTANCE OF TAX IDENTIFICATION NUMBER (TIN)

KNOW ALL MEN BY THESE PRESENTS:

I, ________________________________________________, of legal age, single/married/widow, Filipino, resident of and with postal address at ________________________________________, bona-fide member of the Laguna Prime Multipurpose Cooperative (LPMPC) with Passbook # ____________________, hereby appoint the Laguna Prime Multipurpose Cooperative (LPMPC) with office address at Sacay Grand Villas, College, Laguna or its authorized representative to be our true and lawful ATTORNEY-IN-FACT to do and perform the following acts and things, to wit:

1. To represent Me before the Bureau of Internal Revenue (BIR) as maybe necessary for the purpose of facilitating the process of Tax Identification Number (TIN);

2. To submit, receive or obtain the relevant documents such as but not limited to application of Tax Identification Number (TIN).

HEREBY GIVING AND GRANTING unto my said attorney full power and authority whatsoever requisite or necessary to be done in about the premises and hereby ratifying and confirming all that

By proceeding with membership, you acknowledge that you have read, understood, and agreed to the terms and conditions of this Agreement. You further confirm that you will comply with the requirement to provide a valid TIN as a condition of your membership in the Cooperative.
